Have an 8th grader? Please submit your photos and complete the field trip waivers!
SLIDESHOW PHOTO REQUEST
As part of the 8th Grade celebration ceremony on the last day of school, the PTO puts together a slideshow with pictures of your 8th graders. Please click on the link below, and fill out the information as best as you can. We would love up to four photos: one from kindergarten, one doing what they love, and up to two candid photos with other Galileo students. PLEASE GET PICTURES SUBMITTED BY FRIDAY, MAY 10TH.

- Adult + Minor(s)
- Select “1” for number of minors
- Complete release form (your info and your student’s info)
- Click “Signature” & type signature
- Complete Waiver
AUTO BELAY & BOULDERING AGREEMENT:
- Fill out the form & watch the video with your student
- Have your student take the quiz
- Fill out the 2 signature sections
- Complete Agreement
